He was born on November 22, 1965, in Copenhagen, as Mads Dittmann Mikkelsen, to Bente (née Christiansen) and Henning Dittmann Mikkelsen.
He studied dancing at the Balettakademien in Gothenburg.
In 1996, he made his film debut in the thriller Pusher. He reprised his role in Pusher II (2004).
From 2000 to 2004, Mikkelsen portrayed Allan Fischer in 32 episodes of the TV series Unit One.
In 2006, he played Le Chiffre in the spy thriller film Casino Royale. The film grossed more than $616 million worldwide.
